Vision insurance plans coverage includes*:

  • Routine Examination (100% / $32)

  • Frames* (100% / $30)

    • Non-Collection Frames: $60 Allowance or $110 allowance at Visionworks locations​

    • Additional discount of 20% on any frame overages

  • Standard Eyeglass Lenses (per pair)

    • Single Vision (100% / $25)

    • Trifocal (100% / $46)

    • Aphakic/Lenticular (100% / $72)

    • Bifocal (100% / $36)

  • Contact Lenses 

    • Disposable: Included in Plan Formulary / $85 allowance

    • Conventional (per pair): $85 allowance

    • Additional discount of 15% on any non-collection contact lens overages

  • Medically Necessary Procedures (covered with prior approval / up to $225)

    • Unconditional breakage warranty to repair or replace any Davis Vision laboratory supplied eyeglasses for a period of one year from the date of delivery
